Job Description

Minimum Qualifications:

* 2 years of completed study at an accredited educational institution with coursework completed in behavioral science

* 1 year of experience in a human services, public health, medical clinic, or similar setting

* Demonstrated ability to interact skillfully with a wide variety of people



Preferred Qualifications:

* 4 years of completed study at an accredited educational institution with a degree in social sciences, public health, community health, or similar degree

* Experience working with medical records or databases

* Experience working with PH Doc system



ESSENTIAL FUNCTIONS:

1. Performs Intake, Support and Scheduling: Provides administrative support to program staff. Copies, prints, and routes information needed to support public health programs and client eligibility. Performs Intake functions; collects and documents demographic and other information about clients to determine program eligibility. Maintains other resource lists and documents for public health programs.



2. Enters, Maintains, and Downloads Reports: Prepares and downloads reports in PH Docs, and other public health data systems. Generates client letters and notices coordinating support activities to provide information to clients using various communication methods including; standard mail, electronic mail, text messaging, phone, verbally, or other appropriate methods. Verifies Medical Assistance eligibility of clients and updates systems to reflect accurate eligibility and contact information to include communication with internal and external partners.



3. Conduct follow up with clients and families: Assists public health staff in follow up contacts with public health clients enrolled in a variety of public health programs. As directed, provides general information and referral information to clients and families. As directed generates correspondence and communication via appropriate communication method including; standard mail, electronic mail, text messaging, phone, verbally, or other appropriate methods.



4. Quality Assurance and Quality Initiative: Engage in public health efforts to develop, maintain, and report on quality measures for public health programs. Participate in development of quality assurance and quality initiative tools and surveys. Track responses of clients and providers and assist in development of reporting methods. Develop and maintain tracking of referral sources to public health programs. Assures accuracy of information related to clients and families in public health tracking systems. Work proactively and collaboratively on client engagement and retention in public health programs.



5. Responds to Emergencies: As directed by the public health supervisor, participates in disaster support, communicable disease outbreaks, and investigations in whatever role assigned or designated. Works with communities to provide support in areas of need. Assists with planning, consulting, investigating, and providing assistance.



6. Interacts Positively with Internal and External Customers. Communicates and interacts effectively and respectfully with employees, supervisors, individuals from other organizations, and customers.
