Location: Remote (must reside in NC or SC)
Pay Rate: $40–43/hr

In this role, you’ll serve as a vital link between members, providers, and resources—ensuring coordinated, efficient, and high-quality care across the continuum while improving clinical outcomes and cost effectiveness.
What You'll Do

  • Serve as a key member of a multidisciplinary team, coordinating care, resources, and services for members
  • Conduct comprehensive assessments and collaborate with members, families, and providers on care plans and interventions
  • Develop and manage individualized, member-centric care plans, addressing barriers and support needs
  • Utilize community resources and funding sources to enhance care delivery
  • Monitor member progress, adjust care plans as needed, and maintain appropriate follow-up based on acuity
  • Facilitate care across the continuum (e.g., physician offices, hospitals, rehab, SNF, home care)
  • Educate members to encourage proactive health management and improved outcomes
  • Document all aspects of care management, from assessment through outcome evaluation
  • Identify and outreach to high-risk or high-utilization members as needed
  • Review alternative treatment plans, benefits, and medical necessity of services
  • Support development of policies, procedures, and training when needed
What We're Looking For
  • A current, up-to-date resume
  • RN, RD, or LCSW with 3+ years of clinical and/or case management experience (or MSW with 5+ years in some cases)
  • Active, valid clinical license (NC or compact state)
  • Strong care coordination, assessment, and clinical decision-making skills
  • Experience working with multidisciplinary teams and community resources
  • Knowledge of utilization management and medical necessity criteria
  • Excellent documentation and communication skills
  • Ability to manage multiple cases and adapt to changing priorities
  • Ability to meet compliance requirements
